ICU Consultant - Home Care Visits

About the Client

An opportunity is available for an Intensive Care Consultant to provide home care visits for a young, long-term intensive care at home client in Brisbane. The client is a young woman who was diagnosed with a brainstem glioma at age 14. She later sustained a hypoxic brain injury from a cardiac arrest. Her primary disability is an Acquired Brain Injury, with a secondary disability of a Hypoxic Brain Injury. As a result, she has a prolonged disorder of consciousness, is on a ventilator, has a tracheostomy, and is completely reliant on PEG feeding. She has a complex medical history that also includes non-convulsive status epilepticus, Adam Lang syndrome, and a healing Stage 4 sacral pressure injury. The client is unable to communicate verbally and has impaired decision-making capacity. She lives ant home and requires 24/7 INTENSIVE CARE AT HOME nursing care with CCRN's.

The Role

We are seeking a highly skilled and compassionate Intensive Care Consultant to join a dedicated team of 24/7 INTENSIVE CARE AT HOME Critical Care Registered Nurses (CCRNs) to provide specialized home care. Your role will involve regular home visits to oversee the client's complex medical needs and provide high-level clinical guidance and support to her nursing team.

Key Responsibilities

* Clinical Oversight: Provide expert consultation on the client's intensive care needs, including ventilation, tracheostomy management, and complex wound care.

* Assessment & Monitoring: Conduct comprehensive assessments, including neurological status (GCS and pupillary assessment), fluid status, and pain assessment, to detect any signs of deterioration.

* Medical Management: Guide the management of seizure prevention medications and provide oversight on the administration of rescue seizure medication as needed.

* Intervention & Escalation: Determine if rescue medication has been successful and escalate to definitive medical care if necessary. Use clinical judgment to assess whether medications should be administered or withheld.

* Team Collaboration: Work closely with the CCRNs to ensure seamless care delivery and provide training and support on advanced intensive care procedures.

* Ad-Hoc Support: Be available for consultation on an as-needed basis to assist with troubleshooting equipment or managing clinical changes.

Requirements

* Specialist medical training and qualifications in Intensive Care Medicine.

* Extensive experience with long-term ventilator-dependent patients.

* Proven ability to work autonomously and provide clinical leadership in a home-based care setting.

* Exceptional communication and interpersonal skills to collaborate with the client, her family (Statutory Health Attorney/Plan Nominee), and the care team.

* Current medical registration with AHPRA.
