Aged Care Assessment Team Coordinator

We are seeking a dedicated and experienced Aged Care Assessment Team (ACAT) Coordinator to join our dynamic team at Limestone Coast Local Health Network.
As an ACAT Coordinator, you will be responsible for providing expert nursing and/or midwifery services across diverse health service settings, supported by experience and/or advanced studies.
You will develop from competent to proficient practitioner, ensuring high standards of practice and mentoring less experienced staff.
Key Responsibilities:
- Provide expert nursing and/or midwifery services across diverse health service settings.

- Support the Nursing/Midwife Unit Manager or equivalent in leading nursing/midwifery teams within the ward/unit/service.
- Promote continuity and consistency of care in collaboration with AN/MUM colleagues and management.
- Assist in implementing practice changes and contribute to ward/unit/service management responsibilities.
- Take charge of the development, coordination, implementation, and evaluation of the LCLHN Aged Care Assessment program.
- Provide specialist knowledge and skills in the field of aged care assessment, enhancing team capabilities and ensuring excellence in patient care.
