Aged Care Assessment Team Coordinator

We are seeking a dedicated and experienced Aged Care Assessment Team (ACAT) Coordinator to join our dynamic team at Limestone Coast Local Health Network. As an ACAT Coordinator, you will play a crucial role in providing expert nursing and/or midwifery services across diverse health service settings.
Key Responsibilities- Develop and implement the LCLHN Aged Care Assessment program through acute, residential and community settings.
- Provide specialist knowledge and skills in the field of aged care assessment, enhancing team capabilities and ensuring excellence in patient care.
- Support the Nursing/Midwife Unit Manager or equivalent in leading nursing/midwifery teams within the ward/unit/service.
- Promote continuity and consistency of care in collaboration with AN/MUM colleagues and management.
- Assist in implementing practice changes and contribute to ward/unit/service management responsibilities.
We are looking for a highly skilled and experienced Nurse or Nurse/Midwife with a strong background in aged care service provision. You will have extensive experience in working with individuals/carers who have dementia or mental health problems and their families. Additionally, you will have experience in the supervision of student nurses, enrolled nurses and less experienced registered nurses.
