Senior Hospice Nurse Case Manager

Our healthcare organization is seeking a skilled and compassionate Senior Hospice Nurse Case Manager to oversee a caseload of patients and provide high-quality nursing care.

About the Role:
  • The ideal candidate will have experience in hospice care, with a focus on providing patient-centered care and collaborating with interdisciplinary teams.
  • This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and caregivers.
Responsibilities:
  • Develop and implement comprehensive nursing plans that meet the unique needs of each patient.
  • Provide skilled professional nursing care, including wound management, medication administration, and symptom control.
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
Requirements:
  • Graduate of an accredited registered nursing program.
  • Bachelor's degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • NJ RN Licensure by the NJ State Board of Nursing in good standing.
  • Valid drivers license and reliable transportation.
Preferred Qualifications:
  • Minimum one year general nursing experience with current hospice, home health, medical, surgical, or critical care experience.
  • Experience with pain and symptom management interventions preferred.
