Our Mission:
As a Registered Nurse Case Manager, you will be part of an interdisciplinary team providing compassionate care to patients and their families. You will oversee a primary caseload of hospice patients and provide skilled professional nursing care as prescribed by the physician.
Responsibilities:
- Conduct initial assessments and comprehensive nursing assessments to identify patient needs
- Develop and implement individualized plans of care in collaboration with the Hospice Interdisciplinary Team (IDT)
- Provide education, supervision, and counseling to patients, family members, and staff
- Work closely with other IDT members to ensure all patient and family care needs are met
- Supervise Hospice Aides/Homemakers
Requirements:
- Graduate of an accredited registered nursing program
- Bachelor's degree in Nursing preferred
- Certified Hospice and Palliative Nurse (CHPN) certification preferred
- NJ RN Licensure by the NJ State Board of Nursing in good standing
- Valid drivers license and reliable transportation
Preferred Qualifications:
- Minimum of one year general nursing experience, preferably in hospice, home health, medical, surgical or critical care
- Experience with pain and symptom management interventions
- Ability to work within an interdisciplinary setting
