Our Mission:

As a Registered Nurse Case Manager, you will be part of an interdisciplinary team providing compassionate care to patients and their families. You will oversee a primary caseload of hospice patients and provide skilled professional nursing care as prescribed by the physician.

Responsibilities:

  • Conduct initial assessments and comprehensive nursing assessments to identify patient needs
  • Develop and implement individualized plans of care in collaboration with the Hospice Interdisciplinary Team (IDT)
  • Provide education, supervision, and counseling to patients, family members, and staff
  • Work closely with other IDT members to ensure all patient and family care needs are met
  • Supervise Hospice Aides/Homemakers

Requirements:

  • Graduate of an accredited registered nursing program
  • Bachelor's degree in Nursing preferred
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred
  • NJ RN Licensure by the NJ State Board of Nursing in good standing
  • Valid drivers license and reliable transportation

Preferred Qualifications:

  • Minimum of one year general nursing experience, preferably in hospice, home health, medical, surgical or critical care
  • Experience with pain and symptom management interventions
  • Ability to work within an interdisciplinary setting
