End-of-Life Care Specialist

Job Summary:
We are seeking a highly skilled and experienced senior clinician to lead our end-of-life care team. The successful candidate will have a strong background in clinical assessment, management, and education, with a passion for delivering high-quality patient-centered care.
Key Responsibilities:
- Provide advanced clinical assessment skills and management experience to support high-quality patient-centred care.
- Develop and implement comprehensive end-of-life care plans in collaboration with patients, families, and healthcare teams.
- Conduct staff education, training, and mentoring on shared decision making practices and advance care planning.
- Facilitate advance care planning discussions with patients to document future care preferences.
- Work closely with the Advisor, Shared Decision Making, the Director of Medical Services, and the Chief Medical Officer to ensure the provision of safe and effective person-centred care.
Requirements:
- Normally have at least 10 years' post-qualifying experience.
- Completion of a postgraduate qualification relevant to the position requirements.
- Understanding of the Medical Treatment Planning and Decisions Act (2016) and the Voluntary Assisted Dying Act (2017).
- Current Victorian Driver's Licence essential.
- Australian working rights.
