Case Management Specialist

This role involves supporting individuals to address the underlying causes of their offending behaviour.

Key Responsibilities:
  • Provide information and advice to stakeholders about programs and supports available through the Courts Innovation Program.
  • Manage a caseload of participants with complex and multiple needs, assessing risk and developing case management plans to ensure participants engage with appropriate support and treatment services.
  • Develop and maintain effective relationships with stakeholders to initiate, negotiate, and facilitate linkages and access for participants to a range of services.
  • Prepare accurate progress and final reports to assist with bail and sentencing decisions.
  • Provide court advisory services to inform the court of participant involvement, progress, and outcomes on the program.
  • Liaise with the judiciary, legal profession, and court staff to manage participants within the program and provide advice regarding service availability.
  • Make decisions about brokerage funding expenditure and resource use for participants.
  • Conduct peer reviews and verifications of assessments and other case planning activities to ensure quality assurance and consistency.
  • Maintain accurate records in a computerised database to support program delivery and monitoring.
