Job Title: Integrated Case Management SpecialistDescription:

The Integrated Case Management Specialist plays a pivotal role in delivering high-quality, culturally appropriate services to young people and their families. This involves working closely with internal and external stakeholders to establish strong therapeutic alliances and achieve positive casework outcomes.

  • Develop and implement comprehensive case plans that address the unique needs of each young person and their family.
  • Conduct assessments, interventions, and counseling sessions to support young people in achieving their goals and improving their well-being.
  • Prepare and present information and evidence to the Children's Court, including conducting pre and post-court interviews and liaising with key court stakeholders.
  • Engage with and establish partnerships with community organizations and other service providers to ensure seamless referrals and effective coordination of services.

This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with diverse stakeholders. The successful candidate will be able to think critically, solve complex problems, and demonstrate a commitment to delivering best-practice services.

Requirements:
  • Relevant tertiary qualifications in social work, psychology, or a related field.
  • Proven experience in case management, counseling, or a related field.
  • Strong knowledge of legislative, policy, and procedural requirements related to youth justice.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with diverse stakeholders.
