Job Description:

 

We are seeking a skilled professional to lead patient-centered assessment and discharge planning for individuals admitted to our rehabilitation and care programs.

Effective communication with patients and families is essential, as well as establishing clear goals tailored to individual needs and ensuring timely discharges.

As a leader within the multidisciplinary team, the successful candidate will mentor staff, enhance care coordination capabilities, and contribute to strategies that improve access and flow across the unit.

Key Responsibilities:

  • Lead patient-centered assessment and discharge planning
  • Provide effective communication with patients and families
  • Establish clear goals tailored to individual needs
  • Ensure timely discharges
  • Manage a caseload of complex care coordination cases
  • Liaise with community services and stakeholders to deliver comprehensive care
