Job Description

As a dedicated healthcare professional, you will lead patient-centred assessment and discharge planning for individuals admitted to our Sub Acute Rehabilitation and GEM programs.

The role involves providing effective communication with patients and families, establishing SMART goals tailored to individual needs, and ensuring timely and coordinated discharges.

Key Responsibilities:
  • Managing a care coordination caseload, supporting clients with complex needs, and liaising with community services and stakeholders to deliver comprehensive care.
  • Playing an active role in mentoring staff, enhancing care coordination capabilities, and contributing to access and flow strategies across the unit.
