The role of the Motor Neurone Disease (MND) Care Specialist is a vital component of our Palliative Care team.
We are seeking an experienced and compassionate individual who can enhance the quality of life for people living with MND by promoting timely referrals to palliative care services, providing support to individuals, their families, healthcare professionals, and community health care providers.
This rewarding role requires an individual who can provide coordinated care while navigating changing needs due to progressive symptoms and requirements.
Care Coordination:
- Promote early referral to palliative care services.
- Support development and delivery of education to palliative care service providers.
- Provide coordination between palliative care services, MND clients, and families in the Hume region.
Key Responsibilities:
- Assist individuals with MND and their families to access relevant information and resources.
- Work closely with the MND Advisor and Support Coordinator to ensure effective care pathways.
- Collaborate with healthcare providers to develop and implement personalized care plans.
