Aged Care Transition Specialist

A warm and welcoming onboarding experience is key to a seamless transition into aged care.
As our Onboarding Coordinator, you will be the first point of contact for new residents and their families.
Your role will involve coordinating the full onboarding experience, ensuring a smooth and reassuring transition into residential aged care. This includes responding to enquiries from families with empathy and professionalism, assessing and qualifying prospective residents, and managing all necessary documentation.
You will oversee admissions, conduct personalised facility tours that build trust and confidence, and support families with cost-related questions by offering clear and transparent guidance. Additionally, you will manage entry and discharge procedures with precision and care, while fostering meaningful relationships with residents and their loved ones to create a warm and welcoming environment they can truly call home.
Our ideal candidate will have 2+ years' experience in residential aged care admissions or related onboarding roles. They will present services with clarity, confidence, and a personal touch, possess strong communication skills, and thrive in a collaborative team-oriented environment.
