Enhance Access to Healthcare for Older Persons

The role of a Nurse Practitioner or Transitional Nurse Practitioner is crucial in enhancing access to healthcare for older persons by addressing service gaps within dynamic Models of Care.

Responsibilities include improving health outcomes for older persons by providing timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions while collaborating with other healthcare team members.

As a key member of the healthcare team, you will work closely with patients, families, and carers to provide comprehensive care and support.
