Job Role Overview

This role is focused on delivering responsive outreach case management services to individuals, couples and families who are experiencing homelessness or are at risk of becoming homeless.

About the Job

The Case Manager works directly with clients to address their crisis housing needs and link them to relevant services that enhance their quality of life.

This role requires a high level of autonomy, discretion and cultural sensitivity when working with diverse client groups.

  • Provide case management support to participants in accordance with program goals and objectives.
  • Undertake initial assessments to ensure program eligibility and ongoing assessments as required to meet participant needs.
  • Record and maintain accurate and up-to-date participant information using electronic data systems.
  • Develop and maintain strong partnerships with community service providers through referrals and promotion of relevant programs.
  • Attend meetings, professional supervision and development opportunities to stay current with industry best practices.
  • Contribute to the planning and provision of community education and support initiatives.

To be successful in this role, you will have a degree level qualification in Social Work, Human Services or Behavioural Sciences or a Certificate level 4 or higher qualification in the community sector with relevant experience. You will also possess excellent written and verbal communication skills, as well as the ability to work effectively in a fast-paced environment.
