Crisis Housing Case Manager Family Services Role

Job Description

This position involves working directly with individuals, couples and families who are homeless or at risk of becoming homeless via a walk-in and outreach case management model.

The role provides a responsive outreach service to offer information, referral and advocacy support. Case Managers work with clients to address their crisis housing needs and link them to services to enhance their quality of life and status in the community through access to secure housing options.

  • Provide tailored support to participants in line with program objectives and develop proactive strategies to engage individuals and achieve desired outcomes.
  • Conduct initial assessments to ensure program eligibility, and ongoing evaluations completed as per operational guidelines, utilising the Outcome Star tool to track participant progress and barriers to achieving goals documented in individual plans and at review meetings.
  • Ensure electronic record systems are utilised to capture and maintain participants' personal information in accordance with service agreements, legislation and program policies and procedures.
  • Contribute to the development and maintenance of relevant community partnerships via networking, actively promoting other stakeholders through referrals and promotion of programs.
Key Responsibilities
  • Attend and participate in team meetings, professional supervision and professional development opportunities.
  • Engage in outreach and community engagement activities as required.
  • Contribute to program development and implementation, including participation in planning and provision of community education and support.
  • Comply with legislative requirements (including Workplace Health and Safety, Privacy and Equal Employment Opportunity), service agreements and Quality Management System standards.
Requirements
  • Hold a degree-level qualification in Social Work, Human Services or Behavioural Sciences, OR Certificate level 4 or higher qualification in the community sector plus at least two years experience working within a case management framework in a relevant field.
  • Possess excellent written and verbal communication skills, strong analytical abilities, and a current open driver's licence.
  • Hold a current Working with Children Blue Card check and Disability Services Yellow Card, and provide evidence of right to work in Australia.
