Job Summary:

This position provides direct support to individuals, couples and families who are homeless or at risk of becoming homeless through a walk in and outreach case management model. The role focuses on delivering responsive outreach services that provide information, referral and advocacy support.

Key Responsibilities:
  • Provide case management support tailored to the program target group, focusing on early intervention strategies to engage participants and achieve both program and individual goals.
  • Conduct initial assessments to ensure program eligibility and ongoing assessments as required, utilizing the Outcome Star tool to monitor progress and identify barriers.
  • Record and maintain participants' personal information using electronic data systems in accordance with service agreements, legislation and program policies.
  • Promote community partnerships through networking and referrals, contributing to the development and maintenance of relevant relationships.

Required Skills and Qualifications:

  • Bachelor's degree in Social Work, Human Services or Behavioural Sciences or equivalent qualification with significant experience in case management.
  • Demonstrated expertise in culturally appropriate community work, assessment and referral, with strong analytical and communication skills.
  • Current driver's license and ability to retain necessary certifications (Working with Children Blue Card check and Disability services Yellow Card).
  • Evidence of right to work in Australia.
