Case Manager - Crisis Housing Families

This position is designed to work directly with individuals, couples and families who are homeless or at risk of becoming homeless. The role provides a responsive outreach model to provide information, referral and advocacy support.

The Case Manager will work with clients to address their crisis housing needs and link them to services to enhance their quality of life and status in the community through access to secure housing options.

Responsibilities:

  1. Provide case management support appropriate to the program target group and develop early intervention strategies to engage participants and achieve both program and individual goals.
  2. Undertake initial assessments to ensure program eligibility and ongoing assessments completed in line with work instructions.
  3. Utilize the Outcome Star tool to ensure participants identify their progress and barriers to achieving goals documented in support plans and at reviews.
  4. Maintain electronic data systems to record and maintain participants' personal information in accordance with service agreements, legislation and program policies and procedures.
  5. Contribute to the development and maintenance of relevant community partnerships via networking and referrals.
  6. Attend and participate in meetings, professional supervision and professional development.
  7. Participate in outreach and community engagement activities as required.

Requirements:

  • A degree level qualification in Social Work, Human Services or Behavioural Sciences or Certificate level 4 or higher qualification in the community sector plus at least two years' experience working within a case management framework in a relevant field.
  • Excellent written and verbal communication skills as well as strong analytical skills.
  • A current open drivers licence.
  • A Current Working with Children Blue Card check and Disability services Yellow Card.
  • Evidence of your right to work in Australia.
