Job Summary

We are seeking a highly skilled and compassionate Transition to Adulthood Support Specialist to join our team. As a key member of our team, you will work closely with young people, their families, and other stakeholders to provide tailored support and guidance as they navigate the transition to adulthood.

Key Responsibilities

  • Deliver effective coaching, care coordination, and advocacy to support young people transitioning from out-of-home care
  • Collaborate with key stakeholders, including natural mentors, child safety services, health providers, and community support services, to create a positive experience for young people
  • Mentor young people to help them set and achieve personal goals, finding creative solutions to overcome barriers
  • Provide practical support in housing, employment, health (including mental health), and overall wellbeing to establish a foundation for growth
  • Work sensitively with Aboriginal and Torres Strait Islander people and communities to ensure culturally inclusive services in a safe environment

Requirements

  • A degree in Social Work, Psychology, Social Sciences, or a related discipline
  • A minimum of two years' experience in a human services role, ideally within a multidisciplinary team
  • A solid understanding of human services and care coordination
  • Knowledge of the Child Protection Act 1999, or a willingness to learn
  • Experience in trauma-informed care principles, particularly in assessment, planning, and decision-making for young people
  • Pre-requisites: National Police Certificate, Working with Children (Blue Card), Child Safety and Personal History Screening Check (LCS), and a current driver's licence
