Case Manager Role

The Case Manager will work with individuals and families in partnership to empower them to make an effective change and overcome their identified needs.

  • Provide supports and interventions, based on the program model of care.

Job Responsibilities

Required Skills and Qualifications

COVID - 19 Requirements

  • Immunisation history showing you have received two doses of a Covid - 19 vaccine is encouraged.

Required

  • Current Cert IV in Social Work, Human Services or related field with at least 12-months experience in a case management role.

OR

  • Current graduate university bachelor's degree in social work, Human Services or related field with at least a 10-week-full-time placement in social service field.
  • Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
  • Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.

Additional Requirements

  • Requirement to travel across the region, as per program location.
  • Flexibility to work outside business hours in meeting individual client needs and provide responsive care.
