Client Empowerment Specialist

The Case Manager will work with individuals and families in partnership to empower them to make an effective change and overcome their identified needs.
- Provide supports and interventions, based on the program model of care.
- Provide high quality interactions with clients, consistent with the ATSICHS values and vision.
- Provide case management practices and record keeping that adhere to operational requirements.
- Facilitate a client-led assessment and Care Plan to identify client goals, including the entire family where appropriate.
- Provide targeted interventions and family group meetings to achieve client goals.

COVID - 19 Requirements
- Immunisation history showing you have received two doses of a Covid - 19 vaccine is encouraged.
Required
- Current Cert IV in Social Work, Human Services or related field with at least 12-months experience in a case management role.
OR
- Current graduate university bachelor's degree in social work, Human Services or related field with at least a 10-week-full-time placement in social service field.
- Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
- Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
- Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
Additional Requirements
- Requirement to travel across the region, as per program location.
- Flexibility to work outside business hours in meeting individual client needs and provide responsive care.
