Job Description

A fulfilling opportunity has arisen for a Support Coordinator to join our team in Elderslie. In this role, you will play a crucial part in empowering tenants to live life their way.

As a member of our community team, your primary responsibility will be to provide effective assessment and planning to coordinate and support tenants and household members access to a range of individualized services.

The ideal candidate will possess a Certificate IV in Social Work or Community Services, along with experience in social housing, community services, or a related discipline.

Key Responsibilities Include:

  • Providing effective assessment and planning to coordinate and support tenants and household members access to a range of individualized services
  • Facilitating the provision of effective and individualized support to achieve positive outcomes for tenants and household members
  • Working with high-need, complex clients and liaising with key stakeholders
Required Skills and Qualifications

To succeed in this role, you will require:

  • Certificate IV in Social Work or Community Services
  • Experience in social housing, community services, or a related discipline
  • Ability to facilitate the provision of effective and individualized support to achieve positive outcomes for tenants and household members
  • High-level negotiation skills and ability to liaise with key stakeholders
  • Competence in service assessment and documentation (including service planning)
  • Valid NSW Driver's License
