About the Role

The Care Coordinator plays a crucial role in ensuring high-quality care for our residents. Reporting to the Clinical Care Coordinator and Registered Nurse, this position is responsible for supervising and coordinating care to residents across the home while maintaining individual needs, wishes, and goals.

This is an excellent opportunity for Endorsed Enrolled Nurses or Personal Care Workers to take their career to the next level. We welcome applications from individuals with relevant qualifications and experience.

Key Responsibilities:

  • Provide holistic social and emotional support to residents, ensuring their care needs are met in a person-centred environment.
  • Coordinate care needs across the home, including medication management, nutrition, hydration, and pressure care management.
  • Lead and supervise care on the shift, within designated wings or areas.
  • Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, regulation, and Calvary policies.
Requirements
  • Diploma of Enrolled Nursing (with APRHA Registration to administer medication) OR Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines) OR Certificate III in Individual Support with willingness to undertake training to obtain required qualifications.
  • A high level of commitment, integrity, honesty, and trustworthiness.
  • An effective listener with excellent communication skills, both oral and written.
  • Ability to understand and use information technology and programs, as well as learn new skills as required.
  • A current National police check and working knowledge of WH&S and Infection Control.
  • A desire to join a team committed to providing exceptional care in a mission-based organisation.

Salary Package: We offer a competitive salary package, estimated at $65,000 - $75,000 per annum, depending on experience. This includes NFP salary packaging benefits, discounted health insurance, and gym memberships.
