
Coordinated Access Service Coordinator

This role encompasses the setup, management, promotion, adoption, and ongoing development of coordinated access within the Cape Breton Regional Municipality.
The successful candidate will serve as the secretariat to the governance committee, collaborating with stakeholders to achieve objectives outlined in the Reaching Home Coordinated Access Guide.
Key Responsibilities- Coordinate and regularly convene local governance committee meetings
- Ensure representation on the governance committee is appropriate and effective
- Recommend new committee members as needed and facilitate their integration
- Support timely decision-making to advance CA program goals
- Maintain current Terms of Reference (Roles and Responsibilities) for the committee
- Define articulated coordinated access outcomes, local priority populations, and 12-month governance committee goals
- Onboard new committee members efficiently
- Manage record-keeping for the committee
In addition, this role involves:
- Implementing and ensuring compliance among primary users
- Broadening understanding and adoption among secondary users
- Creating visual materials to enhance broader understanding and adoption
- Fostering trust in HIFIS and its importance
- Providing assistance with troubleshooting for users
Furthermore, this role also entails:
- Developing a system map of services for homeless/at-risk of homelessness populations
- Establishing the map as a precursor to future Coordinated Access Shared Resources Inventory
- Exploring options to make the map available online/searchable/designated for community users
The ideal candidate will possess experience working with vulnerable populations, excellent communication and organizational skills, and ability to work independently and collaboratively.
A highly organized individual with exceptional attention to detail, they will also be proficient in Microsoft suite of programs and applications.
The successful candidate will demonstrate servant leadership, community-centricity, high level of integrity, confidentiality, and accountability.
