The role of this position is to deliver high-quality social work services to individuals living with HIV and their loved ones as part of a comprehensive care package.
Key Responsibilities:- Conduct thorough psychosocial assessments, develop individualized care plans and provide ongoing support to patients undergoing treatment for HIV.
- Offer counselling services to newly diagnosed individuals, those dealing with the challenges of living with HIV, or requiring adjustments to their care needs, including addressing stigma related to HIV.
- Collaborate closely with a multidisciplinary team to ensure seamless patient care, facilitate discharge planning and coordinate community-based interventions.
- Establish partnerships with local organizations and build strategic relationships within the HIV sector to enhance service delivery.
