HIV Social Work Specialist

At Alfred Health, we strive to provide comprehensive psychosocial assessments, reviews, and facilitate timely quality care plans for patients living with HIV and their families/significant others.
About the Role:- Deliver social work services to patients living with HIV and their families/significant others as part of The Alfred Statewide HIV Service.
- Collaborate with multidisciplinary teams to support patient care and discharge planning.
- Establish and maintain community linkages and partnerships, especially in the HIV sector.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments and reviews and facilitate timely, quality care plans.
- Provide counseling to individuals newly diagnosed with HIV, those adjusting to illness, or facing changes in care needs, including addressing HIV stigma.
- Offer inpatient social work to HIV inpatients to ensure continuity of care when required.
Requirements:
- Advanced social work skills with preferably 3+ years post-qualifying experience and a commitment to ongoing professional development.
- Experience in health settings with an understanding of issues facing diverse patient populations and hospital admission/discharge planning.
