HIV Care Specialist

We are seeking a skilled professional to fill the role of HIV Service Social Worker Grade 2. This is a permanent, full-time position within our multidisciplinary team.
The successful candidate will provide comprehensive psychosocial assessments and reviews to patients living with HIV and their families/significant others.
Key Responsibilities- Develop and implement timely and quality care plans in collaboration with healthcare professionals.
- Deliver counseling services to individuals newly diagnosed with HIV, addressing stigma and other challenges.
- Collaborate with a multidisciplinary team to support patient care and discharge planning.
- Foster community linkages and partnerships to ensure continuity of care.
- Advanced social work skills with preferably 3+ years post-qualifying experience and a commitment to ongoing professional development.
- Experience in health settings with an understanding of diverse patient populations and hospital admission/discharge planning.
- Ability to work in fast-paced acute or outpatient hospital environments and crisis-responsive community contexts.
- Critical thinking and problem-solving skills.
- Excellent communication and interpersonal skills.
- Strong organizational and time management skills.
