Job Role: Social Worker in HIV Service
About the RoleWe are seeking an experienced and skilled social worker to join our team in the HIV Service. As a social worker in this role, you will work closely with patients living with HIV and their families to provide comprehensive psychosocial assessments and develop timely care plans.
- Supporting HIV social work outreach clinics and collaborating with multidisciplinary teams to ensure seamless patient care.
- Providing counseling services to individuals newly diagnosed with HIV, addressing stigma, and facilitating timely care planning.
- Collaborating with HIV Senior Social Workers and other disciplines to support patient care and discharge planning.
This role requires advanced social work skills, preferably 3+ years post-qualifying experience, and a commitment to ongoing professional development.
Key Responsibilities- Provide comprehensive psychosocial assessments and reviews.
- Facilitate timely, quality care plans.
- Counsel individuals newly diagnosed with HIV or facing changes in care needs.
- Develop and maintain community linkages and partnerships.
- Consult with HIV Senior Social Workers and other disciplines as needed.
This role also involves working in fast-paced acute hospital environments and crisis-responsive community contexts. You will need to have a good working knowledge of hospital settings and electronic medical record systems.
