As a key member of our multidisciplinary team, you will provide comprehensive psychosocial assessments and reviews to facilitate timely, quality care plans for patients with HIV.


Key Responsibilities
  • Offer individualized counseling to patients facing changes in care needs, including addressing HIV stigma.
  • Collaborate with healthcare professionals to support patient care and discharge planning.
  • Develop and maintain community linkages and partnerships within the HIV sector.
  • Consult with senior healthcare professionals as needed.
  • Provide inpatient social work services to ensure continuity of care for HIV patients.

Qualifications and Skills
  • Advanced social work skills with at least 3 years post-qualifying experience and a commitment to ongoing professional development.
  • Experience in health settings with an understanding of diverse patient populations and hospital admission/discharge planning.
  • Ability to work effectively in fast-paced environments and crisis-responsive situations.
  • Knowledge of relevant population-specific considerations, including HIV stigma, LGBTIQ+ issues, and cultural diversity.
  • Ability to lead a portfolio, clinic, or program within the team.
  • Strong working knowledge of electronic medical record systems and hospital/community settings.
