
Care Coordinator for Individuals Living with HIV

The Social Worker Grade 2 role is part of the HIV Outward Program, delivering care and support to individuals living with HIV who have complex care needs. This position collaborates with a multidisciplinary team, including Nursing, Medical, and Mental Health Clinicians.
This challenging role involves:
- Conducting psychosocial assessments and reviews using clinical reasoning to formulate timely quality care plans.
- Offering counseling to those newly diagnosed with HIV, adjusting to illness or changes in care needs.
- Developing and maintaining community linkages and partnerships, particularly in the HIV sector.
The ideal candidate will possess advanced social work skills, experience working with diverse patient populations, and a commitment to ongoing professional development. Key qualifications include:
- 3+ years post-qualifying experience as a Social Worker
- Demonstrated experience in health and an understanding of issues impacting the lives of people living with HIV.
- Ability to work within a fast-paced acute or outpatient hospital environment and crisis-responsive community context.
- Knowledge of hospital admission and discharge planning processes.
In addition, the successful applicant will have:
- A good understanding of adjustment to illness and counseling support
- The ability to develop and lead a portfolio, clinic, or program within the team
- Good working knowledge of hospital and community settings and systems
