Job Role: Intensive Case Manager

Overview: We are seeking an experienced and dedicated professional to join our team as an Intensive Case Manager. In this role, you will provide high-quality intensive case management services to young people aged 16-24 experiencing homelessness.

Key Responsibilities:
  • Support Young People: Effectively support a caseload of young people through a healing-oriented framework.
  • Develop Advanced Case Planning: Develop advanced case planning and crisis interventions to meet the needs of young people presenting with multiple and complex needs, including risk assessments and safety plans.
  • Assist Accessing Accommodation: Assist young people to access ongoing accommodation options, particularly supported accommodation, family reconciliation, private rental, shared housing and social or transitional housing in accordance with the case plan.
  • Maintain Knowledge: Maintain an up-to-date knowledge of relevant external service providers to ensure an integrated service response to young people.
Requirements:
  • Qualifications: Qualification in social work, youth work, community development or a related tertiary qualification.
  • Experience: Extensive experience (4 years) working in the community services sector.
  • Critical Skills: Demonstrated experience working within a case management framework, understanding of mobile, street based and assertive outreach interventions.
  • Personal Qualities: Strong passion for working with young people and ability to actively and assertively engage young people experiencing complex needs.
