Job Description:

The Outreach Practitioner plays a crucial role in providing health services to marginalized and vulnerable adults in Suffolk. They establish connections with hard-to-reach clients, reconnecting those who have fallen outside mainstream services back to necessary care.

This vital member of the multidisciplinary team engages in client case management, need assessment, and risk evaluation, delivering outreach services to high-risk groups.

Key Responsibilities:
  • Manage client cases effectively
  • Assess needs and identify potential risks
  • Deliver outreach services according to best practices
Required Skills and Qualifications:

Our ideal candidate possesses excellent communication skills, a compassionate approach, and strong problem-solving abilities. They must be well-versed in NICE guidelines and evidence-based best practices, with experience working with diverse populations.

They should be able to liaise effectively with partners serving asylum seekers, refugees, black and minority ethnic groups, ex-offenders, gypsies and travellers, homeless individuals, and migrant workers.
