Mental Health Support Coordinator

$32-$35 p/h | Salary Sacrificing Available
HealthWISE is seeking a compassionate and motivated Support Coordinator to join our Ipswich-based wellbeing team. In this rewarding role, you'll provide high-intensity support coordination for people living with severe and complex mental illness—helping them build resilience, access services, and work towards recovery in a safe and supported way.
This position is ideal for someone with lived experience of mental health recovery or who has supported someone through it, and who is passionate about making a difference in the lives of others.
**About the Role**:
- Deliver community-based, person-centred support
- Develop tailored Safety and Support Plans
- Work collaboratively with clinicians, GPs, and service providers
- Support clients to navigate the health system and improve social connectedness
- Provide outreach with some travel and overnight stays required
**About You**:
- Certificate IV or Diploma in Mental Health (or equivalent)
- Personal experience with mental health recovery (own or as a carer)
- Strong communication and interpersonal skills
- Organised, flexible, and able to manage competing priorities
- Driver’s licence, Blue Card (WWCC), and National Police Check required
