Job Overview
As a Community Care Support Worker, you will be working as part of a multidisciplinary care team to support young people in transitioning out of care and into sustainable independent living.
Key Responsibilities:
- Provide essential support and assistance to young people to develop and increase their capacity and links to the community.
- Facilitate and contribute to the development of activities that promote effective independent living skills.
- Work collaboratively with the care team to review and ensure each young person progresses towards their identified goals.
- Support young people in developing parenting skills for expectant/young parents.
- Mentor and coach young people to achieve their goals in accordance with Participation and Therapeutic Care Plans.
Requirements
To be successful in this role, you will need to have:
- A minimum Certificate IV qualification in a relevant discipline (e.g., Youth Work, Social Work, Social Science, Counselling).
- Holding and maintaining an approved First Aid and CPR Certificate.
- Preparedness to undergo a Psychological Assessment.
- Possession of a current valid Driver's License, willingness to drive.
- Safe Environments for Children and Young People Certificate.
- Willingness to participate in an out-of-hours on-call roster.
