Community Care Support Worker Role

Job Overview

As a Community Care Support Worker, you will be working as part of a multidisciplinary care team to support young people in transitioning out of care and into sustainable independent living.

Key Responsibilities:

  • Provide essential support and assistance to young people to develop and increase their capacity and links to the community.
  • Facilitate and contribute to the development of activities that promote effective independent living skills.
  • Work collaboratively with the care team to review and ensure each young person progresses towards their identified goals.
  • Support young people in developing parenting skills for expectant/young parents.
  • Mentor and coach young people to achieve their goals in accordance with Participation and Therapeutic Care Plans.

Requirements

To be successful in this role, you will need to have:

  • A minimum Certificate IV qualification in a relevant discipline (e.g., Youth Work, Social Work, Social Science, Counselling).
  • Holding and maintaining an approved First Aid and CPR Certificate.
  • Preparedness to undergo a Psychological Assessment.
  • Possession of a current valid Driver's License, willingness to drive.
  • Safe Environments for Children and Young People Certificate.
  • Willingness to participate in an out-of-hours on-call roster.
