
Pathways Coordinator

The Aboriginal Pathways Practitioner plays a vital role within the Community and Family Services division, collaborating closely with key stakeholders to promote the wellbeing of young people.
This position involves providing efficient and quality initial assessments for Aboriginal and Torres Strait Islander children, triaging referrals to mitigate child protection risks and enhance safety for vulnerable children and their families.
The successful candidate will be responsible for:
- Enhancing communication and understanding with key stakeholders to support timely and effective service responses.
- Promoting and developing strong partnerships with government and non-government agencies to achieve best outcomes for families.
- Working in partnership with stakeholders across child protection, education, and human services systems to identify and implement responses for vulnerable families.
- Providing consultation on referrals received by the CFSS Pathways Service for Aboriginal and Torres Strait Islander children and families.
Key Outcomes and Accountabilities Include:
- Providing specialist recommendations and advice on culturally responsive strategies.
- Developing local, regional, and state-wide networks with government and non-government agencies.
- Engaging in supervision, case discussion, and reflective practice sessions to support decision-making and innovative thinking.
- Supporting the development of culturally responsive practice within the CFSS Pathways Service.
- Collaborating with community partners to identify service gaps and determine joint solutions.
In addition, the successful candidate must hold an appropriate Social Work qualification from a recognised tertiary institution and possess a current Australian issued driver's licence. National Police Check and Employment-related Screening Check are also required.
