About the Role

We are seeking a dedicated and compassionate Home Care Support Worker to join our team at Aged Care Resumes. As a Home Care Support Worker, you will play a vital role in providing high-quality care and support to individuals in their homes, enabling them to maintain their independence and quality of life.

Key Responsibilities
  • Provide in-home and community support to individuals with mental health issues, disabilities, elderly individuals, and those requiring community care.
  • Assist with personal care, domestic assistance, respite, community access, meal preparation, and other daily tasks.
  • Monitor client progress, report any concerns or changes, and document accurate and relevant information in client files.
  • Establish and maintain strong relationships with clients, their families, and other healthcare professionals.
Requirements
  • Hold a qualification in Individual Support, Aged Care, Disability, or Community Care.
  • Have a valid Police Check, Working With Children Check, and NDIS Worker Check.
  • Hold a valid First Aid and CPR Training Certification.
  • Have access to a reliable car and a valid Driver's License.
  • Be willing to do short services.
