About the Role

Australian Unity is seeking a dedicated and compassionate Support Worker to join our Home Health program. As a Support Worker, you will play a vital role in delivering in-home support services to our aged care customers, empowering them to live independently and safely in their own homes and communities.

Key Responsibilities

  • Provide personal care and support to customers in their homes, including assistance with daily living activities, transportation, and social outings.
  • Build relationships with customers and their families, conducting regular observations and providing ongoing support and training.
  • Adhere to health and safety guidelines and policies to ensure the well-being of customers and staff.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.

Requirements

  • Current Australian Driver's License and ownership of a reliable, comprehensively insured vehicle.
  • Flexibility and availability to work rosters between Monday to Friday.
  • Proficiency in an additional language is valued but not mandatory.
  • Previous experience in disability support, community services, mental health, or a related field is highly regarded.
