Community Care Support Specialist

The Afterhours Community Connect Counsellor position is part of a multidisciplinary team supporting carers who contact the Carer Gateway Service after hours, experiencing sudden and significant changes in their caring role.
This position supports carers via phone between 8pm-8am Monday – Friday, 24 hours Saturday, Sunday and Public Holidays on a rotating roster.
- Receives inbound and outbound calls to provide therapeutic support, coping strategies, activation of care and/or safety plans for family carers.
The service is not a crisis line, and this position is expected to assist with warm referrals.

Main Responsibilities:- Supports carers via phone experiencing emotional distress.
- Provides therapeutic support, coping strategies, and activates care and/or safety plans.
