Job Title: Community Care Specialist

We are seeking a skilled and dedicated professional to provide high-quality care across all age groups in our Integrated Family Health Service (IFHS) team in the Buller Region. This is an exciting opportunity to contribute to improved rural health outcomes.

About the Role:

  • Deliver comprehensive, patient-centered care within the primary care setting to enhance health and wellbeing outcomes.
  • Provide culturally competent care, prioritizing the needs of Māori as tangata whenua to achieve equity in health outcomes.
  • Support and empower individuals and whānau to improve health literacy and overall wellness.

Key Responsibilities:

  • Undertake holistic assessments within culturally appropriate frameworks.
  • Develop collaborative and documented care plans that support continuity of care.
  • Work closely with community stakeholders and the interprofessional team to address unmet healthcare needs.

About You:

  • New Zealand Registered Nurse with a current Annual Practising Certificate (APC).
  • Computer literacy, including experience with Indici and Microsoft Office Suite.
  • Strong initiative and ability to think critically.
