A bit about the role

This is an exciting opportunity to join our team as a Home Care Worker, where you'll play a vital role in supporting participants with their daily activities. As a valued member of our team, you'll have the chance to make a meaningful impact on people's lives and contribute to our mission of providing person-centered care.

What you'll do

You'll be working closely with participants to understand their individual needs and develop personalized support plans. Your key responsibilities will include:

  • Providing companionship, meal preparation, domestic assistance, and medication management
  • Supporting participants with their daily activities and promoting independence
  • Collaborating with the participant's support network to ensure seamless care delivery
  • Contribute to the continuous improvement of care provided to participants

Requirements

To succeed in this role, you'll need:

  • Certificate lll in Mental Health, Disability, Aged Care or Home and Community Care (desirable but not essential) or equivalent working or lived experience
  • A current valid Driver's License and a registered, insured vehicle
  • Police Records Check (completed within the last 12 months)
  • Working with Children Check
  • Manual handling training is desirable, but you will receive training if needed
  • Rights to work in Australia
  • Availability and willingness to use personal smartphone
  • NDIS Workers Screening Check
  • NDIS Worker Orientation Module Certificate – free online course
