About the Role

We are seeking a talented Care Coordinator Specialist to play a vital role in supporting elderly people in the community, enabling them to live independently and with dignity in their own homes.

As a permanent full-time Care Coordinator Specialist, you will be responsible for managing a caseload of community-based elderly individuals with Home Care Packages. Your primary focus will be on assessing their needs, developing personalized care plans, and managing budgets according to program guidelines, ensuring each customer's unique requirements are met.

You will also onboard new customers and proactively manage existing ones with a person-centered approach. Collaboration with our Clinical, Scheduling, and Service Delivery teams is crucial to delivering comprehensive care.

In this role, you will have the flexibility to manage your own schedule each week while working autonomously from home and within the community. You will visit customers in their homes across Wollongong and the Sutherland Shire regularly, therefore you will ideally be located in or close to this region.

About You

  • Prior experience in community-based case management is highly desirable.
  • Experience with Home Care Packages is also beneficial.
  • A current driver's license and reliable vehicle with minimum Third Party insurance are essential.
  • You must possess strong communication skills, both verbal and written, as well as demonstrated empathy and compassion.
  • Excellent problem-solving abilities and critical thinking skills are required, along with good organizational and time management skills.
  • Cultural competence and the ability to work with diverse populations are necessary, as are advocacy skills to ensure clients' needs are met.
  • Collaboration and teamwork skills to work effectively with interdisciplinary teams are also crucial.
