We are seeking a dedicated Health and Wellbeing Coordinator to support the health and wellbeing of our residents within our Supported Independent Living services.

Key Responsibilities:
  • Developing, implementing and evaluating holistic health and wellbeing programs for residents in our supported independent living facilities
  • Collaborating with multidisciplinary teams to ensure comprehensive care and support for residents
  • Monitoring and reporting on resident's health and wellbeing, and adapting programs as needed
  • Organising and facilitating social, recreational and therapeutic activities to promote active and engaged lifestyles
  • Providing guidance and support to residents and their families to enhance their health, independence and quality of life
  • Maintaining accurate records and documentation in line with regulatory requirements
Requirements:
  • Relevant tertiary qualification in an allied health, nursing or community services discipline
  • Extensive experience in a similar role within the aged care or disability support sector
  • Exceptional interpersonal and communication skills with the ability to build strong relationships with residents, families and multidisciplinary teams
  • Demonstrated expertise in developing and implementing health and wellbeing programs that promote independence and quality of life
  • Strong organisational and problem-solving skills with the ability to prioritise and manage competing demands
  • Commitment to providing person-centred care and upholding the rights and dignity of vulnerable individuals
