Empowering Individuals and Fostering Meaningful Connections

We are seeking a dedicated Support Worker to provide tailored interventions within a developmental framework, enabling NDIS participants to address their needs.

You will assist in delivering activity-based work and daily living supports, such as accompanying participants to medical/specialist appointments. Our goal is to empower participants with disabilities to develop meaningful relationships and connections.

The Role

  • Accompany people with a disability to medical or specialist appointments, promoting their health and wellbeing.
  • Support participants to engage in activities and tasks that promote community independence, employment opportunities, and age-appropriate experiences.
  • Assist participants in accessing activities and outings in the community, enhancing their social connections and overall quality of life.
  • Provide direct care assistance to each participant in accordance with their NDIS plan, working closely with D&J Connect's Support Coordinator.
  • Work in accordance with medication administration policies, work health and safety procedures, and participants' rights policies, ensuring confidentiality and privacy.
  • Monitor the safety and wellbeing of participants, reporting any concerns to D&J Connect.
  • Offer one-to-one or small group training and support to meet individual participants' needs.
  • Maintain accurate documentation of services provided.

Key Requirements

  • Certificate III in Individual Support (Disability) preferred.
  • Australian or New Zealand citizenship or relevant working visa documentation.
  • Suitable vehicle insured for third-party fire and theft.
  • Valid C Class driver's license or above.
  • Recent police check completed within the last six months.
  • Working with Children check clearance.
