Closed - We're seeking a professional to play a key role in helping individuals navigate their aged care journey.

As a Community Services Intake Officer, you will be responsible for guiding individuals through the aged care process. This involves conducting assessments, developing tailored care plans and coordinating services across various programs to ensure each person receives the right level of support for their needs.

  • Conduct client intakes and assessments
  • Develop, review and update individual care plans
  • Monitor changing client needs
  • Maintain accurate records and ensure compliance
  • Build strong relationships with clients and internal teams

You will need to work collaboratively as part of a team and have a warm, compassionate approach to delivering high-quality services.

To be successful in this role, you will require:

  • Experience with client intake, assessment and care planning
  • Solid understanding of consumer-directed care principles
  • Strong admin, IT and organisational skills
  • Excellent communication and interpersonal skills
