
Care Coordinator

We are seeking a highly skilled and compassionate individual to fill the role of Clinical Guidance Specialist. The selected candidate will be responsible for providing expert guidance and support to care and allied health staff, ensuring the delivery of high-quality, person-centred care that meets the unique clinical, social, and emotional needs of our residents.
Key Responsibilities:- Providing expert clinical guidance and decision-making to promote best possible nursing practice.
- Leading registered nurses and providing coaching, mentoring, and supervision.
- Actively involved in delivering care and services to residents, including promoting their physical, emotional, and social well-being.
- Participating in quality management systems, including data analysis and trends.
- Promoting and coordinating clinical documentation, including assessment, care planning, and evaluation.
- Reviewing progress notes and other documentation daily to ensure accuracy and completeness.
- Managing incidents when they occur, and implementing strategies to prevent future occurrences.
- Supporting residents, families, and teams in coordinating case conferencing and interdisciplinary collaboration.
- Relevant experience in a senior nursing position, with a strong focus on leadership and mentorship.
- In-depth understanding of AN-ACC requirements and standards.
- Current registration with AHPRA, with a commitment to ongoing professional development.
