Care Manager

The Care Manager - Home Care is a key position within our organization, responsible for leading the achievement of high-quality clinical and care services in the Southern region. This role will be based at our Central Office, coordinating resources and activities to achieve person-centered care for clients/care recipients that is responsive to their assessed needs.
Key Responsibilities- Ensure all clients in the region have a current and holistic assessment and a service plan that manages all risks associated with their assessed needs, within their service budget and with a health-promoting focus.
- Actively engage with the community within the region, promoting Southern Cross Care as a Provider of Choice for Support at Home services to older people.
- Act as an advocate for a client/care recipient to assist with making informed decisions.
- Monitor the quality of comprehensive care planning strategies to ensure the client's quality of life and independence is optimized, while managing risks and duty of care.
- Ensure all assessments and documentation meet legislative requirements.
We are looking for individuals who will be the difference in the lives of our residents and clients. You will work amongst a multi-disciplinary team to provide integrated care that promotes healthy aging and well-being, social engagement, prevents frailty and avoidable decline, and achieves reablement for clients to optimize their ability to live better in their own home.
The successful candidate will have the following skills and experience:
- Bachelor of Nursing, or recognized equivalent, and current registration and practicing certificate with AHPRA as a Registered Nurse.
- Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009).
- Current and unrestricted driver's license & own registered and insured car.
- Strong organizational and time management skills.
- Excellent interpersonal and communication skills, as well as the ability to build positive relationships with clients and their representatives.
- Effective decision-making ability, including the ability to identify potential risks and escalate as appropriate within the organization.
- Proficient computer and keyboard skills, including the use of community aged care systems/processes (e.g., MyAged Care, ACAT, RAS, etc.).
- Ability to demonstrate integrity, calmness, and personal resilience.
- Passionate about person-centered care, enabling clients to maintain maximum independence, you will be able to seek creative solutions in your approaches to overcome any barriers.
