The Sustaining NSW Families Program Clinical Coordinator plays a pivotal role in the operational management and leadership of the nurse-led home visiting program for vulnerable families.

Job Description:

This position involves managing new referrals, undertaking initial client engagement, and providing clinical consultation to team members. The successful candidate will lead case reviews and relieve clinical staff during short periods of leave, ensuring seamless delivery of services to families.

Key Responsibilities:
  • Develop and implement strategies to improve service outcomes and quality
  • Liaise with stakeholders to ensure effective collaboration and communication
  • Maintain accurate records and reports to inform decision-making
Required Skills and Qualifications:
  • Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA) and relevant management tertiary qualification or equivalent work experience
  • Extensive post-registration experience in Child & Family Health Nursing (CFHN)
  • Experience working with vulnerable families and a demonstrated understanding of early intervention, including sustained home visiting, prevention, and community development
