Senior Clinical Coordinator for Vulnerable Families

We are seeking a dedicated Senior Clinical Coordinator to lead our Sustaining NSW Families Program, delivering high-quality care to vulnerable families from pregnancy to two years old.

This role offers the opportunity to make a meaningful impact on the lives of children and families in need. As a seasoned Child and Family Health Nurse with extensive experience in home visiting programs, you will be responsible for operational management, clinical leadership, and team coordination.

Your key responsibilities will include:

  • Managing new referrals to the SNF Service and undertaking initial client engagement and clinical assessments.
  • Providing clinical consultation for team members, leading case reviews, and relieving clinical staff during short periods of leave.
  • Leading a multi-disciplinary team to deliver better outcomes for families in Northern Sydney Local Government Areas.

To be successful in this role, you will possess:

  • Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA) and relevant management tertiary qualification or equivalent work experience.
  • Extensive post-registration experience in Child & Family Health Nursing (CFHN).
  • Experience working with vulnerable families and a demonstrated understanding of early intervention, including the theoretical basis that underpins sustained home visiting, prevention, and community development.
  • Experience in managing a team and knowledge in strategic and operational service planning and quality improvement processes.
