Nurse-Led Home Visiting Program Manager

The Clinical Coordinator will oversee the operational management and leadership of a nurse-led home visiting program for vulnerable families.
This role requires a Child and Family Health Nurse who can manage new referrals, undertake initial client engagement and clinical assessments, provide clinical consultation, lead case reviews, and relieve staff during short periods of leave.
Key Responsibilities:- Manage new referrals to the program
- Undertake initial client engagement and clinical assessments
- Provide clinical consultation for team members
- Lead case reviews
- Relieve clinical staff during short periods of leave
- Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA)
- Relevant management tertiary qualification or equivalent work experience
- Demonstrated commitment to professional development
- Extensive post-registration experience in Child & Family Health Nursing (CFHN)
- Experience working with vulnerable families
- Demonstrated understanding of early intervention, including the theoretical basis that underpins sustained home visiting, prevention, and community development
- Experience in managing a team
- Knowledge in strategic and operational service planning and quality improvement processes
