Clinical Leadership Role in Home Visiting Program

This position oversees the operational management and leadership of a nurse-led home visiting program. Key responsibilities include managing new referrals, undertaking initial client engagement and clinical assessments, and providing clinical consultation for team members.

The Clinical Coordinator is part of a multi-disciplinary team that delivers outcomes-driven care to families in Local Government Areas.

  • Lead a team to deliver high-quality services and improve outcomes for vulnerable families.
  • Manage a team and contribute to strategic service planning and quality improvement processes.
  • Collaborate with healthcare professionals to provide excellent care to families.

Candidates will need to meet the following criteria:

  • Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA) and relevant management tertiary qualification or equivalent work experience.
  • Extensive post-registration experience in Child & Family Health Nursing (CFHN).
  • Demonstrated understanding of early intervention, including theoretical basis for sustained home visiting, prevention, and community development.
