We are seeking a highly skilled and compassionate individual to fill the role of Supported Living Facilitator. As a Supported Living Facilitator, you will work closely with individuals who require support in their daily lives to help them achieve their goals and live independently.
You will be responsible for providing person-centered care, supporting individuals in everyday living situations, and enabling them to make informed decisions about their own lives. Your role will involve working collaboratively with the individual, their family, and other support networks to develop and implement individualized plans that meet their unique needs and goals.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and their families.
