Job Description:

Our ideal candidate will have a strong background in nursing with a focus on assessment, care planning, and case management. Key Responsibilities:

  • Conduct thorough assessments of patient needs and develop personalized care plans in collaboration with the interdisciplinary team.

  • Monitor patient progress and adjust care plans as needed to ensure desired outcomes.

Requirements:

  • Current AHPRA registration.
  • Bachelor's or Master's degree in nursing.

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of an interdisciplinary team.
